Provides a group environment of recreation, socialization, a meal and skill building in a supervised environment, for older adults and specialized populations. Some programs may offer evening/overnight hours.
Assisted living services include personal support, homemaking, security checks or reassurance and care coordination.
Attendant Care Staff provide assistance with Activities of Daily Living (ADLs) to adults with physical disabilities in their home, work or educational environment. ADLs include but are not limited to assistance with personal hygiene, rising and retiring, meal preparation, toileting and ventilator management. Attendant Services include Outreach (scheduled between 6 am and Midnight), traditional Supportive Housing (provided 24/7 in a supportive housing apt or townhouse or congregate setting) and Mobile Supportive Housing (provided 24/7 within a geographic area).
Offers assistance with routine household activities such as light house cleaning, laundry, ironing, and preparing meals.
Nutritionally balanced meals delivered by volunteers who provide a social contact and a safety check for the client. May include hot meals, frozen meals, or fruit/vegetable baskets.
Staff and/or volunteers from multiple disciplines provide health care, palliative pain & symptom management, emotional and practical support to those with life-limiting illness or end of life care.
Door-to-door rides for eligible individuals to attend medical and therapeutic appointments and social activities. Includes physical rides or subsidies for public transit or taxi. May include a trained staff/student or volunteer to accompany the individual.
